Dr. DeCapite and staff are dedicated to providing you with a pleasant visit and results that you’re proud to show off. Our friendly administrative staff is ready to help you with questions about scheduling, financial policy and insurance, to make that part of the process as simple as possible.

Please download, print and fill-out our New Patient Forms. After you have completed the forms, please make sure to bring them on your first visit to our office. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it. Please do not email the forms, as that is generally not secure.

Children

The first visit should be just after your child’s third birthday. This usually a short visit and involves very little treatment. We may ask you to sit in the dental chair and hold your child during the examination. You may also be asked to wait in the reception area during part of the visit so that a relationship can be built between your child and your dentist.

We will gently examine your child’s teeth and gums. X-rays may be taken (to reveal decay and check on the progress of your child’s permanent teeth under the gums). We may clean your child’s teeth and apply topical fluoride to help protect the teeth against decay. We will make sure your child is receiving adequate fluoride at home. Most important of all, we will review with you how to clean and care for your child’s teeth.

Insurance

At Dr. DeCapite’s office, we make every effort to provide you with the finest care and the most convenient financial options. To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. We are an In-Network provider for many insurance plans.
